How to write a vendor report people can actually use

The most useful vendor reports answer the same few questions. Copy this and fill it in:

What I ordered (compound, size):
Batch / lot number:
Ordered on / arrived on:
Shipping: carrier, packaging, cold pack (yes/no), condition on arrival:
COA: provided? does the batch number match the vial?
Support: did you need them? how fast and how helpful?
Would you order again, and why:

Facts beat feelings — "arrived in 9 days, no cold pack, COA batch matched" helps the next person far more than "great vendor!". Link the vendor when you post so the thread shows up with their profile.
